Click Here to download the appropriate viewing software II. Step Two: Fill in forms accurately. The Enrollment Form is a brief form that gives us all the essential information we need about your child. The Statement of Health must be filled out by a doctor or health personell and have a stamped transcript of immunizations attached. The health form is not necessary until the second visit. III. Step Three: Bring them in! Bring in your completed enrollment form and drop off the children for fun at Club 4 Kids!
